Avoid  unnecessary injections  . Almost all the  medicines are available as oral preparations  .When you administer drugs orally  the  unnecessary components  will not get absorbed  . But  when you administer  the drug  by parenteral route by injections   all the components  will get absorbed   and  can cause  injury to internal organs . Hence  it is  better  to avoid  injections  until  it is absolutely indicated .Almost all medicines are associated with one or other complications  either  mild  or  moderate  or severe .

1)Incidence of  sickness following an injection

One  day   I  saw  a  whole  batch of   50 medical students in a  medical college hospital  sitting  in the class room when  they were  supposed  to  go  to the ward .Everyone  was  looking sick . I asked  them  why they did not go to the ward .I  got  the reply   that  they  were sick and is suffering from fever . When  I asked  how  all 50  students  can have  fever   simultaneously they  told  that they had  fever . They  got some injection which was provided free of  cost . When I asked  what  was the  injection  and  what was a the injection  and  why it was  given only in the later part of  December  no one  knew .

Discussion-  An injection is said to be valid  for  one  year  only  that is from January 1 st to December 31st.  The  next batch  of new  injections  will be given  from 1st January  to 31st December . The  injections  that will expire  by December 31st  will  be  provided  free of  cost  to  Medical people and  Nursing people   Mostly  the Medical and  nursing  students  will go  to get  these  injections .  If  these  injections are   to  be  discarded  they  companies  has  to undergo lot of  procedures   permissions  which  will cost a lot .   In order to  avoid  these the  remaining   injections  are  provided free of cost  to  the  innocent students. The students who got these injections  also act  as  promoters  for these injections

2)Incidence  of  Chronic  Diarrhoea –

I received a  call from my  student from Tirunelveli . Her  child  was  having diarrhea  for  more than one month .  I had  come  across many such cases  over a period of time.

Discussion-  I asked the mother  if  there was  any previous  salient history . She said    the baby was given some oral  medication   which contained  live virus  . The  diarrhea  had started  exactly after the incubation period  .

3)Incidence  of Sudden infant Death Syndrome 

The in incidence of  babies  feeding well and  put  of  bed  and sleeps  well are found dead in the evening of night . The incidence  has increased in the recent times

Discussion.The  condition is  known as  Sudden infant Death Syndrome  . The healthy babies  will be found  dead  in the bed  . There will be no cause found. 

4)Incidence  of  Anaphylaxis

A 5 year old boy  died suddenly  following an injection in Salem .

Discussion-   On post Mortem although   the  findings  suggested  the  death is due  the  injection , it was  decided that  the child  died  due to Sepsis .

5)Incidence  of  sudden death

Two children died  following one  injection  near  Dindugal

Discussion-The incidences  between the administration of injection and the death were analyzed. The children had taken a snack (Bajji) in between. It was inferred  that the death was not due to  injection.

6)Incidence of  thrombosis

Many people  had developed thrombosis  following an injection

Discussion-   As injection contains foreign protein .it may induce coagulation process.

7)Incidence of  Acute  Demyelinating Encephalomyelitis(ADEM )

Many cases of  ADEM were  reported  following a particular injection .Many cases  were reported  at Coimbatore  hospitals . This  was followed by an injection administered  to children

Discussion-  ADEM will be associated  with neurological symptoms  

8)Incidence of sudden death following exercise

Discussion-   of late  many adults  are  seen  collapsing suddenly following exertion or  exercise . They  had history  of  receiving  particular  injection. 

9)Incidence of sudden death following an  injection

Many young adults  have  died  suddenly at an young age,

Tanjore -One of my friend‘s father   went and  got injected  in Tanjore. He  died suddenly   by next day morning due  to cardiac arrest .

Pollachi –  a husband and wife  who  goes  to walking near Venkatesa colony  was  not found  .Both got injected  and  died  by next day.

Pollachi – Two school teachers aged  between 35  to 39 years died  with in two days .

Pollachi – A house  owner’s wife  aged about 60 years   died  with two days of injection

Coimbatore -A driver aged about 30 years  at Coimbatore 

Sabarimala -A person went  to Sabarimala  following  injection  died  there.

10)  Incidence of  Arthritis  with thromboembolic  manifesattions

Arthritis ( Joint pain) with  thromboembolic  manfestations

Discussion – many people  are developing arthritis , skin manifestations  and  embolic  /thrombus  following  certain injections  or  subclinical infections.

POINTS  TO NOTE –

1)More  complications  are reported  after  a particular injection by the health workers  .

2)Before  few years if  the  complications are  observed  for  a particular injection  , the  alternative  injections  will be available  . but now  such facilities are not available.

About  us-  Few  doctors  who had come across  the above  adverse  events  had  decided  not to advice  these injections  as they felt they are not necessary . The following incidences resulted in initiation of the concept of monitoring the health, treatment, drug intake, diet habits and provide suggestions to the patients.
1. Autism –Autism was believed to be caused by MMR (Measles, mumps and Rubella) vaccine by earlier studies .Later it was said that it was not proved by further studies . It is not known if there is any conflict of interest in those further studies  . As the cause said to be not known it is better to avoid unnecessary drugs or other offending agents. In our patients especially children we have not come across any child with autism. But the incidence of Autism is on the rise among children. It is important to find out the cause and avoid it. Until the cause is known avoid all suspected agents. There is an argument that  this drug say X is given to  millions of  people  does all of them get affected . This is a foolish argument . For example if one is allergic to penicillin it cannot be argued that penicillin is given to millions of people do all of them get anaphylaxis. Similarly  some people are allergic to prawns . If some one get gastrointestinal upset or anaphylaxis fter  eating prawn it cannot be argued   that prawn is eaten by millions of people do all of them get  gastrointestinal upset or anaphylaxis.
2. Resistance to Antibiotics -Use of higher antibiotics for treating infections at the initial stage results in development of antibiotic resistance .Hence if the treatment fails the patient becomes resistant to all antibiotics. One doctor argued that whatever best that is available in the market is given to his patients. This wrong attitude will predispose to development of resistance to antibiotics.
3. Wrong attitude by patients -Patients expect and want immediate cure. If the patient is not cured immediately the Doctor is stamped as inefficient or unlucky . This is the factor which forces one  to prescribe higher antibiotics, multiple drugs, use of steroids etc.
4. Steps to prevent spread of cancer cells – In one of our patients we did not advice biopsy, when cancer (malignancy) was suspected .We advised to the remove the involved part. The chance of complete cure is more. If the part is incised the malignant cells will be spilled out and carried in the blood stream to other parts predisposing to secondaries. Later even if the cancer mass is removed they will spread and develop secondaries and the prognosis will be bad. We have come across many patients who got complete cure. (At the same time this can be considered as a mistake by consumer protection council. Hence only a suggestion will be given regarding the options for the treatment ).There is a small risk of the removed part showing a non malignant condition. The patients has to decide on their own and we are not responsible for the outcome.
5. Wrong food habits had resulted in many disorders and even deaths .The incidence is on the rise among children. We will give the advise regarding problems associated with various food items.
6. Contraindications to drugs – Many drugs are contraindicated when the patient is suffering from certain disorders. Long term monitoring will be helpful in avoiding of prescribing those drugs to them. For example aminoglycosides are contraindicated in renal failure when the blood urea, creatinine are elevated. Beta blockers are contraindicated in diabetics as they will mask the symptoms of hypoglycemia. May drugs are contraindicated when the liver function tests are abnormal.
7. Observer variation –When more than one doctor is monitoring the health problems of observer variation can be avoided. A point missed by one doctor can be picked by other. Always get a  second opinion.
8. Early referral to appropriate higher centers. When there is a health problem the patients are wandering to many health centers. They land up in wrong treatment centers at many times. It is better to go to correct centers which is best for that condition .We aim to guide them to correct centers where the facilities are available.
9. Early intervention –A female patient who is a known case of cardiac disorders developed chest pain with sweating in the morning after her son went to his office. When she informed this to her son he told that he will come back by evening and take her to a doctor. The mother was waiting with a hope that her son will come and save her .The son came by evening and took her to a doctor and got some injections .ECG was not taken to suspect angina or myocardial ischemia. Later she died with in few hours. The chest pain with sweating was a symptom of insufficient blood supply to the heart .She should have been taken to a cardiologist immediately. But for about ten hours she was not attended by anyone and later attended by a general doctor who had not given a correct guidance but also provided a false assurance. This incidence hurt us a lot and was an initiative to start this programme. Just a guidance in the morning would have saved a life.
10. Food records-Many lives were lost due to wrong food habits. Recently we heard of a student who ate a commercial food product developed abnormal clinical symptoms and took self medication. With in minutes he died. Many dangerous ingredients are added to the commercial food products and results in even death. But these incidences are not brought to the notice of the public .Even when it is brought to the notice, the parents or students ignore them. Recently the children of few famous actors in Tamilnadu lost their children due to wrong food habits .But nobody had learnt any lessons from those incidences.
